Suraksha Dental Care provides dental treatment and related healthcare services through our clinics and affiliated providers. Our services include but are not limited to:

• Orthodontic treatments
• Oral surgery
• Periodontal treatments

2. Patient Responsibilities

2.1 Accurate Information

You agree to provide accurate, current, and complete information about yourself as prompted by our registration forms and to maintain and update this information to keep it accurate, current, and complete.

2.2 Treatment Compliance

You agree to follow the treatment plans and instructions provided by our dental professionals and to keep scheduled appointments or provide adequate notice of cancellation.

2.3 Payment Obligations

You agree to pay for all services rendered at the time of service or according to the payment plan agreed upon with our office.

3. Appointments and Cancellations

We require at least 24 hours notice for appointment cancellations or rescheduling. Failure to provide adequate notice may result in a cancellation fee.

4. Payment Terms

All fees for services are due at the time of service unless prior arrangements have been made. We accept:

• Cash
• Credit/Debit Cards
• Insurance (where applicable)
• Health Savings Accounts

For insurance payments, you are responsible for any amounts not covered by your insurance provider.

5. Privacy and Confidentiality

We are committed to protecting your privacy and the confidentiality of your personal health information. Our Privacy Policy explains how we collect, use, and protect your information.

6. Limitation of Liability

Suraksha Dental Care shall not be liable for any direct, indirect, incidental, special, or consequential damages resulting from the use or inability to use our services, even if we have been advised of the possibility of such damages.
